Aging & Community Services

The Regional Services team is responsible for a wide-range of activities designed to improve the quality of life at the community level.

Community Transitional Services

GCADRC serves as single point of entry into the long-term care service system and can help alleviate the frustrations many consumers and their families experience when they need to obtain information and access to supports and services.

Are you, or know of someone who is, transitioning from a living facility back into the community?

GCRPC help people transition from facilities back into their homes, connecting them with resources they may need to make the transition successful.
